The problem could be because of he being diabetic. Diabetes Mellitus is known to adversely affect nerves and also could contribute to his penis being semi-hard.

Although he is being treated for diabetes, is his blood glucose level being regularly monitored? Are the levels within normal parameters? This is very important for his sexual function and his general well being as well.
