Thanks for a really great question. I know that you ask something that other young women may want to know the answer to.
You need to be complimented on using a condom with spermicide, and you are right - all the changes in your body would not suggest you have an STD.
What happens when a girl first has intercourse, the skin that is rubbed may get slightly swollen and bleed easily. We have many, many blood vessels in our genital area, right on the surface of the skin, so it is not unusual for there to be swelling and some bleeding.
When the opening is enlarged a little, because the penis is larger than most anything else that goes into the vagina (i.e. tampons or spermicide from a tube) it is not unusual for the area to be a little sore and even for the skin that covers the area partially to be pushed aside or to tear a little. It is not something to be concerned about. It will heal, and probably by the time you read this message, your skin will be back to normal. The hymen gets stretched or moved some, but will remain there since the only way it completely gets removed is a delivery of a baby through the opening which clears all the lose skin out.
